As Nurse Practitioners, we are trained to practice evidence-based medicine and exercise strict diagnostic stewardship. We know that screening asymptomatic populations without clinical indication leads to over-diagnosis, unnecessary interventions, and avoidable patient harm.

That is why the Department of Defense’s recent policy shift has clinicians across the country deeply concerned. Defense Secretary Pete Hegseth announced a new directive requiring annual testosterone screening for all active-duty male service members age 30 and older.

While initiating Testosterone Replacement Therapy (TRT) remains voluntary under the policy, this blanket screening mandate directly defies established endocrinology guidelines, risks overwhelming healthcare resources, and exposes otherwise healthy young adults to serious iatrogenic risks.

Here is a clinical breakdown of why this policy fails the standard of evidence-based practice — and what it means for healthcare systems and patient outcomes.

1. Direct Violation of Clinical Practice Guidelines

Both the Endocrine Society and the American Urological Association (AUA) maintain unambiguous, long-standing mandates against routine, population-wide screening for testosterone deficiency. As advanced practice providers, the diagnostic flaws in this blanket mandate are immediately clear:

The Symptom Prerequisite

According to Endocrine Society Clinical Practice Guidelines, a diagnosis of hypogonadism requires both biochemical evidence and distinct clinical symptoms (e.g., unexplained osteopenia, severe fatigue, or low libido). Testing asymptomatic 30-year-olds inherently creates a high statistical probability of false positives.

Diurnal Fluctuation & Stress Variables

Serum testosterone is highly volatile. Accurate assessment requires a minimum of two separate, early-morning (7:00–10:00 AM) fasting blood draws. Active-duty troops routinely face acute sleep deprivation, extreme physical exertion, and operational stress — all of which cause transient, non-clinical drops in testosterone. A single annual screen cannot differentiate between a true endocrine pathology and a tough week in the field.

 

⚠ CLINICAL NUANCE

A single low reading in a fatigued, sleep-deprived service member is not a diagnosis — it's a prompt to repeat the test under controlled conditions. Reflexively confirming and treating on one draw is where the false-positive cascade begins.

 

2. Evidence-Based Alternatives: The NP Approach to Hormonal Optimization

Before jumping straight to exogenous hormones, NPs know that optimizing a patient’s metabolic health can safely and naturally reverse mild to moderate declines in testosterone. Addressing the root causes of metabolic dysfunction preserves the Hypothalamic-Pituitary-Gonadal (HPG) axis without systemic shutdown.

The Aromatase Cycle and Metabolic Weight Loss

Adipose tissue contains high concentrations of the enzyme aromatase, which actively converts circulating testosterone into estradiol.

      Clinical goal: Losing just 10% to 15% of total body weight can significantly reduce aromatase activity, decreasing estrogen conversion and naturally driving baseline free testosterone levels back up.

      NP intervention: Counsel patients on a sustainable, moderate caloric deficit. Avoid extreme crash diets, as prolonged starvation states elevate cortisol and actively suppress luteinizing hormone (LH) secretion.

      Nutritional guidance: Ensure adequate dietary intake of healthy monounsaturated and saturated fats (avocados, olive oil, nuts), which serve as the essential cholesterol backbones for steroidogenesis.

Progressive Resistance Training

Physical exercise is a highly effective, non-pharmacological stimulus for transient testosterone spikes and long-term metabolic health.

      Heavy compound lifting: Prioritize multi-joint, compound resistance movements — squats, deadlifts, and overhead presses. These recruit large muscle groups and high-threshold motor units, eliciting a much greater acute hormonal response than isolation exercises.

      Volitional fatigue: Workouts should utilize challenging loads within a 1-to-8 or 8-to-12 repetition range to maximize mechanical tension.

      The cortisol danger: Advise patients to avoid excessive, long-distance endurance training. Chronic, exhausting cardio floods the system with cortisol, an endocrine disruptor that directly antagonizes testosterone synthesis.

 

3. A Critical Warning on “Other Therapies”: Estrogen Blockers

In fitness subcultures and some wellness clinics, estrogen blockers (such as aromatase inhibitors like anastrozole, or SERMs like clomiphene) are frequently utilized off-label to artificially boost testosterone. As clinicians, we must exercise extreme caution here.

By blocking estrogen or stopping its production, these drugs trick the brain into thinking estrogen levels are low. The pituitary gland responds by pumping out more LH and FSH, which temporarily signals the testicles to make more testosterone.

⛔ CASE FOR CAUTION — CRASHING ESTROGEN

Estrogen is absolutely vital for male health. Crashing a patient's estradiol levels destroys bone mineral density, triggers severe joint pain, negatively impacts lipid panels, and increases cardiovascular risk.

Major endocrinology guidelines do not support the use of aromatase inhibitors for age-related or lifestyle-induced low testosterone in men. Relying on these off-label pharmaceuticals instead of lifestyle interventions introduces unvetted clinical risk.

 

4. Advanced Laboratory Analysis: What to Order and How to Interpret

When an asymptomatic patient is flagged with low testosterone on a routine screen — or presents with vague, non-specific symptoms like fatigue — a single “total testosterone” value is clinically insufficient. Diagnostic stewardship means evaluating the entire biochemical layout before labeling a patient deficient.

The Screening & Confirmation Pathway

Order Total Testosterone (TT), drawn 7:00–10:00 AM, fasting.

What to Order: The Comprehensive Hormone Panel

      Total Testosterone (TT): Measures bound and unbound hormone. It's the standard starting point, but can be misleading — a patient with a seemingly normal TT can still have clinical hypogonadism if bioavailable levels are depleted.

      Sex Hormone-Binding Globulin (SHBG): Roughly 60% to 70% of circulating testosterone is bound to SHBG and biologically inactive. Obesity, insulin resistance, and high-carbohydrate diets drop SHBG; aging and hyperthyroidism raise it.

      Free & Bioavailable Testosterone: Free testosterone (1% to 2% of total) plus albumin-bound testosterone make up the bioavailable pool — the only portion that can enter cells and exert effects. If SHBG is high, TT can look normal while Free T is profoundly low.

      LH & FSH: Elevated LH/FSH with low testosterone indicates primary (testicular) failure. Low or inappropriately normal LH/FSH points to secondary (hypothalamic/pituitary) dysfunction, which is often reversible.

      Prolactin: Elevated prolactin suppresses GnRH, causing secondary hypogonadism, and screens for a prolactin-secreting pituitary tumor. Evaluate via head MRI if significantly elevated (>20–30 ng/mL) or if TT is extremely low (<150 ng/dL).

How to Interpret: Differential Diagnosis Framework

Once confirmatory labs return, use this matrix to categorize the patient's condition and determine whether the path forward is permanent medical management or reversible lifestyle intervention.

When interpreting borderline cases, do not guess at bioavailable levels. Use the Vermeulen formula to calculate free testosterone from the interaction of transport proteins — input the patient's Total Testosterone, SHBG, and Albumin into a validated clinical calculator.

A calculated free testosterone level below roughly 6.0–6.5 ng/dL (or 65 pg/mL, depending on lab reference ranges) confirms a true biochemical deficiency — even if the total testosterone number appears falsely preserved due to high SHBG.

 

5. Exposing Patients to Iatrogenic Disability Risks

Initiating TRT — or unapproved pharmaceutical interventions — in a young, healthy population carries profound long-term pathophysiological risks. For active-duty troops, these medically induced complications can result in permanent, service-connected disabilities.

      Secondary Polycythemia & Vascular Events: Testosterone aggressively stimulates erythropoiesis. When hematocrit exceeds 54%, hyperviscosity drastically elevates the risk of DVT, pulmonary embolism, and ischemic stroke — a preventable stroke can leave a 31-year-old with permanent neurological deficits.

      HPG Axis Suppression & Irreversible Sterility: Exogenous testosterone triggers a negative feedback loop on the hypothalamus and pituitary, shutting down endogenous LH and FSH. While often reversible, prolonged TRT can permanently damage testicular Leydig cells, causing irreversible infertility and permanent hypogonadism.

      Exacerbation of Sleep Architecture: TRT is heavily documented to trigger or severely aggravate obstructive sleep apnea by altering central respiratory drives, often tethering a young patient to a lifetime CPAP machine.

 

6. The Bottom Line for APPs

Because these screenings and subsequent treatments are directed by military healthcare channels, any adverse outcome — a vascular event, permanent sterility, or severe sleep apnea — legally qualifies as a service-connected injury. Taxpayers will ultimately fund lifelong VA disability compensation and medical retirements for conditions that were entirely iatrogenic.

Population-wide screening without clinical indication drives false positives, unnecessary confirmatory testing, and a pipeline into lifelong TRT for men who were never truly deficient. As NPs, our role is to advocate for guidelines-based care: repeat and confirm before diagnosing, evaluate the full hormone panel — not TT alone — and lead with reversible lifestyle interventions before exogenous hormones. More testing does not equal better health.

      Two separate, fasting AM (7–10 AM) draws — diurnal variation invalidates a single afternoon result.

      Total testosterone < 300 ng/dL, confirmed on a second fasting AM draw.
